Book An Appointment
Appointments can be scheduled by video or in-person with Dr. Collard. House call appointments are available but limited. If you need an appointment in your home, please schedule and specify your request when you book the appointment. Your requested appointment is not confirmed until you receive an email and/or text message confirmation.
Cancellation Policy: We request that you provide 24 hours notice of cancellation or request to reschedule an appointment. In the event you no-show or do not provide 24 hours notice, you may be charged a $50 cancellation fee. We are a small practice and do not overbook. This policy allows us to be available to provide quality care for patients.
