Step 1: Getting to know you as a person

I’m interested in more than just your medical history! To optimize your project, our initial assessment is detailed and will cover how you currently eat, sleep, stress and move. I want to know not only my patients’ goals and motivations but also the hurdles and obstacles they face every day. Some people start a project after a new diagnosis or encouragement from their physician. Others are looking to relinquish some of their dependence on medications, some are looking to age gracefully without disease but many are just looking for a solution to stop feeling lousy. I don’t believe in a universal approach and knowing you better will help individualize your project.

Step 2: Data collection

This step requires collecting your complete past medical history along with your physical exam, laboratory findings, biomarkers and body composition, and then applying risk assessment algorithms to better predict health outcomes. We want to be as comprehensive as possible. However, we do try to minimize unnecessary labs and expense so we are glad to review recent labs from your physician. Here, we’re looking at your current markers of health, where you’re potentially heading and any warning signs to address in your project.

Step 3: Formulating a project

Putting it all together! Let’s have an honest conversation about where you are and where you are heading with your current health. This comprehensive approach will identify specific areas on which to focus, and in what order. Every project is unique and individualized to each patient.

Here, we formulate the project to move you closer to your goals and a more optimal health state. Depending on your health status, goals and interests, we will develop your project based around lifestyle research to address the most impactful changes you can incorporate specifically for YOUR health.

Step 4: execution and follow through

The most important step of your project. Everyone has a different starting point, but we will apply a multi-pronged and individualized approach that includes a focus on education (on disease and interventions), support to harness your motivation and skill development to ensure your success back at home.

You will be held accountable to the project. There will be follow up along the way to ensure we are on track to reach our goals. When we finish the project, ideally you will have established a new health baseline and habits to carry you forward! We will conduct an assessment to see how far you’ve come, and talk about ways to improve further. Then, 6 months later, you will be welcome back for a complimentary check-in on your independent, healthier lifestyle!

Our Guarantee: We will partner on your personal journey to implement effective and sustainable lifestyle practices for better health. While outcomes may vary, we guarantee you will receive thorough, quality care, and be treated with compassion and respect.

Financial Policy: Rooted Medicine Wellness Project is a direct pay practice. Payment is expected in full at the time of service. You may also pay at the time of appointment confirmation. We accept cash, checks, Visa, Mastercard, Discover and American Express credit and debit cards. Although we do not contract with insurance companies, upon request, we will provide you with a form to submit to your insurance company.  According to the benefits and specifics of your policy plan, they may reimburse you directly.  We have opted-out of Medicare; office visits are not reimbursable for Medicare or Medicaid.
